Below is the medical release form. This must be completed and printed out before your first day of class. If you need a doctor’s note, please make sure you obtain one and bring it to your first day of class. 

If the medical is not completed prior to class, you will be asked to reschedule to another date. 

When completed, please print and bring to class. Your instructor will want a copy.
